1. In order to fill and sign the applications forms, you should have Adobe Reader installed on your device. If you do not have Adobe Reader, you can download it for free here: www.get.adobe.com/reader/. For instructions on how to use the signature feature, see here: https://helpx.adobe.com/ca/reader/using/fill-and-sign.html Note that once you have added your signature to the form and saved, you can no longer make edits to the form. Make sure all fields are filled in before adding your signature. Alternately, you may print out the forms, fill and sign with a pen and then upload scanned copies (pdf only).
2. Please ensure that both biological parents (and/or other legal guardians) are present since both signatures are required.
3. Lastly, please have the following mandatory documents ready to upload prior to filling in the application form:
   
   • Copy of student’s Canadian birth certificate showing parents’ names, OR non-Canadian birth certificate, certified English translated birth certificate (if language other than English), AND passport (if not born in Canada)
   • Supporting documentation for Legal Residency of Parents Form (e.g., copy of parent’s Canadian passport, citizenship card, permanent resident card or work permit.)
   • Supporting documentation for ordinary residency of BC (e.g., copy of parent’s BC Care Card, BC Driver’s Licence or BC Services Card.)
   • Copy of student’s most recent report card (Grades 1-12) or progress report from pre-school / kindergarten
   • Copy of any relevant reports from specialists, such as psycho-educational assessment (if applicable)
   • Copy of any relevant court orders pertaining to custody or guardianship of student.
   • Copy of student’s immunizations
   • Doctor’s note and emergency procedures instructions for life-threatening allergies/illness (if applicable)

Applications require a non-refundable fee that can be paid after submitting the application. You will be directed to our online payment form after completing the application process. If you have already paid then skip this step.
